Week 1: New Practice Approaches

This was the first week and the initial stage of the practicum practice. The emphasis was on the new practice approaches in clinical nursing practice. Currently, nurses are working in an intricate and challenging environment. As such, it is appropriate to acquire practical experiences and theoretical knowledge to enable professional nurses to brace for the challenges and complexities of this current highly demanding health care system and to ensure the provision of quality health care services to patients (Sherwood & Barnsteiner (Eds.), 2021). The challenges and complexities necessitate the adoption of new practice approaches in nursing practice. This practicum experience was crucial in exposing me to the problems encountered by health care settings in care delivery and made me appreciate the need of embracing new practice approaches to help in navigating the health care challenges and complexities and enhance patient outcomes.

This practicum practice is essential in providing me with an opportunity to acquire new knowledge and skills and enhance my ability to remain updated with the current changes in the health care system. Keeping abreast with the technological changes in health care is essential for nurse practitioners to remain up-to-date in their effort to deliver efficient care to patients. One of the remarkable changes that have revolutionized the nursing field is the utilization of evidence-based practice (EBP). EBP has significantly informed patient care provision in monumental ways. Therefore, nurses should be ready to adopt the EBP and other emerging changes in health care delivery to help them in informing clinical reasoning and decision making, support ideas with evidence, and health promotion (LoBiondo-Wood & Haber, 2021). Taken together, this week’s practicum experience exposed the challenges and complexities of the health care system and how they can influence nursing practice. I have also learned how EBP can be used to inform nursing practice.

The focus in the second week was interprofessional collaboration. Interprofessional collaboration entails the coordination of health care teams drawn from various departments within the health care setting to work together in the health care delivery.  The multiple health care professionals come with different specialties, skills, and disciplines and team up to provide in-inclusive and quality care to patients, families, and the community (Ansa et. al., 2020).  The practicum experience exposed me to the importance of interprofessional collaboration in patient care delivery. The current health care system is complex and challenging, which needs diverse health care providers to work collaboratively to provide safe, quality, and efficient care (Goldsberry, 2018).  I observed that interprofessional collaboration has the benefit of reducing errors. Usually, errors emanate from ineffective communication or communication breakdown among providers. Such gaps in communication are abridged when providers work collaboratively, thus, minimizing medical mistakes and enhancing patient safety (Goldsberry, 2018).

When the health care needs of patients are approached from different perspectives by diverse disciplines, it yields positive and exceptional outcomes.  This is practical because interprofessional collaboration integrates diverse knowledge, skills, resources, and talents to address patient health care issues (Karam et al., 2018).  During my practicum experience, I partnered with providers from various disciplines such as physicians, dieticians, counselors, pharmacists, social workers, educators, and fitness coaches. The collaboration centered on effective communication, sharing ideas about the health conditions of patients, and seeking possible solutions to those conditions.  Conditions such as childhood obesity often require the input of different providers from different specialties.  Interprofessional collaboration is important to both patients and the health care providers since one provider cannot solely address all the health care needs of a patient.  Ultimately, interprofessional collaboration plays a critical role in the health care system including fostering safe, quality, and efficient provision of health care.

Professional Capstone and Practicum Reflective Journal
Assessment Description
Students are required to submit weekly reflective narratives throughout the course that will culminate in a final, course-long reflective journal due in Topic 10. The narratives help students integrate leadership and inquiry into current practice.

This reflection journal also allows students to outline what they have discovered about their professional practice, personal strengths and weaknesses, and additional resources that could be introduced in a given situation to influence optimal outcomes. Each week students should also explain how they met a course competency or course objective(s).

In each week’s entry, students should reflect on the personal knowledge and skills gained throughout the course. Journal entries should address one or more of the areas stated below. In the Topic 10 graded submission, each of the areas below should be addressed as part of the summary submission.

New practice approaches
Interprofessional collaboration
Health care delivery and clinical systems
Ethical considerations in health care
Practices of culturally sensitive care
