IOM Future of Nursing Report

The United States has been characterized by fragmentation in different sectors of its health care system. Consequently, various barriers hinder the citizen from accessing quality, affordable, and accessible health care services. This has also contributed to a discrepancy between the health services offered and the nation’s health needs. Sullivan (2018) notes that stakeholders believe that it is essential to expand the roles of health care professionals and providers, including nurses, to make affordable and quality health care services accessible to the diversified population of the United States. In collaboration with the Robert Wood Johnson Foundation (RWJF), the Institute of Medicine (IOM) released a report to shape the future of nursing, transform the health care system, and address the issues hindering accessibility to affordable, safe, and quality health care. The purpose of this paper is to describe the work of the RWJF Initiative leading to the IOM report recommendations, the key messages in the report, and how they have transformed nursing leadership, education and training, workforce, and nursing practice.

The Work of the Robert Wood Foundation Initiative

In the report “The Future of Nursing: Leading Change, Advancing Health,” IOM envisioned transforming the health care system whereby prevention and primary care are the key drivers with interprofessional coordination and collaboration being demonstrated in the provision of affordable, high-quality, and accessible health services. The RWJF approached the IOM to produce an action-oriented blueprint report to reconceptualize the nursing roles and shape the future for the nurses (Sullivan, 2018). The RWJF launched The Future of Nursing: Campaign for Action campaign to implement the IOM report recommendations.

The committee also held various meetings, including public gatherings, to derive the content of their message in the report’s topics. Nurses comprise the majority of the health care professionals in the United States. They also spend the greatest time in providing patient care. The Robert Wood Foundation Committee Initiative spent much time studying the prevalent issues affecting nurses. They studied, analyzed, and reviewed the collected data to developthe issues that prevail in the nursing profession and education. By recognizing the unique roles nurses play in the health care system, the studies assisted in formulating the recommendations of the IOM report to improve both advanced and general health care, to transform the nursing profession and the health care system at large.

The Four Key Messages in the IOM Report

IOM acknowledged the unique roles played by nurses and, after extensive deliberations, came up with four key messages presented in the report. These are the nurses should practice to the full extent of their licensure, training, and education, nurses should achieve higher training and education levels through bettered education systems, nurses should be partners with other health care professionals in redesigning the health care system, and improved information infrastructure and better data collection to achieve effective workforce planning (Rekha, 2020). In the first key message, the report outlines that taking advantage of the full education capacity and unique roles nurses playwould ensure that Americans access high-quality health services. The report further recommends removing barriers limiting nurses to work only in acute care settings. Nurses bridge the gap between access to care and health coverage. Nurses are encouraged to become health team leaders, informaticians, care coordinators, health coaches, and primary care providers. The report recommendations have seen systemwide success from nurses acquiring more knowledge, skills, and competencies and practicing in various care settings, includingcommunity levels. Cherry and Jacob (2019) note that nursing responsibilities and roles have changed since the IOM report was released, with professional nurses possessing more core competencies in interprofessional collaboration, critical thinking, knowledge, assessment skills, use of information technology, and awareness of disease prevention and health promotion.

In the second message, IOM recognizes that improving the education system ensures that the current and future nurse generations continue to deliver patient-centered, safe, and quality care across all settings. Advancing education training equips nurses with more professional autonomy, diagnostic skills, critical thinking, decision-making, and assessment skills, enabling them to deliver safer and high-quality care, responding to the full needs of their clients (Sánchez-Gómez et al., 2019). Nurses have continued to advance their education, as reflected by a report by the American Association of Colleges of Nursing (2019) that states the proportion of nurses with a baccalaureate degree and above is 56%, compared to 49% at the time the IOM report was released.

The third message encourages the transformation of nursing leadership, with nurses partnering more with physicians and other health care professionals. Partnering with other professionals enables nurses to identify problems, devise improvement plans, track goals over time, and make required adjustments to realize the set objectives. Consequently, nurses deliver safer, holistic, high-quality, and patient-centered care, improving health outcomes and satisfaction. Nurses have continually demonstrated mutual collaboration and respect with other health professionals, working skillfully and in tandem to improve patient experiences.

Lastly, the fourth key message speaks to the necessity for better data on the healthcare workforce. Understanding the composition and numbers of various professionals enables planning for changes required to transform the health care system. Data on the current workforce assists in projecting the future workforce needs. Gaps in the workforce data availability hamper nurses’ preparation for practice.

State-Based Coalitions

State-based coalitions are composed of several partners and stakeholders. These include social justice organizations, health education centers, local school systems, workforce organizations, libraries, business groups, foundations, among others, all working together to build healthier communities through nursing.These coalitions form strong networks that leverage the local community’s power to make and maintain advancements in health care. Depending on the local and geographical needs, the action coalitions develop strategic plans to implement the recommendations of the IOM report and realize the objectives of the campaign. The state-based coalitions also research to identify the best possible models to deliver quality and safe health care services.

The California Action Coalition

The California Action Coalition (CA AC) spearheads initiatives and offers a statewide unified voice for the nurses in California. CA AC was launched in late 2011 after the IOM report was released. CA AC has placed great efforts on partnerships, leveraging the potential of California citizens to access high-quality care offered by nurses. CA AC also focuses on various initiatives related to the IOM report recommendations on leadership, education, and practice (California Action Coalition [CA AC], 2021). Mary Dickow, Statewide Director for CA AC, asserts that the coalition has made considerable academic progression. The coalitions ensure that nurses engage in lifelong learning and wanted to double the number of nurses with a doctorate by 2020. Nurses have also been equipped as effective leaders to address and fill current and future problems and positions. Nurses also practice to the full extent of their education and training. As CA AC develops and implements strategies to meet the report recommendations, the health of the Californians has been improved continually.

Barriers to Advancement

Lack of enough funds is a significant barrier that hampers the progression of state-based coalitions. Adequate financing is required to continually improve activities, advance and fully promote agendas, and mobilize resources to achieve the set objectives. Financial constraints have great implications, including low remuneration to staff, including nurses, which would demotivate them, leading to an unwillingness to work to one’s full potential. Nursing advocates continue to encourage volunteers and donors to continue with their tireless efforts to support the coalition’s activities.

Conclusion

To fully achieve the recommendations of the IOM report, the US needs to train nurses to handle diverse and emerging health needs continually. Nurses should continue embracing team-based approaches in delivering care. The responsibility and power to deliver accessible, sustainable, and quality care does not lie solely on the nurses. Collaboration with other professions and larger groups such as educational institutions, licensing bodies, and philanthropic organizations have the power of achieving the set goals and objectives of the IOM report.
