Basic care and comfort are basic things that each one would want to feel at any point in life. Nurses in any healthcare institution promote health and wellness for clients while offering comfort and competent care to increase safety (Langhammer et al., 2018). “Basic comfort and Care” are one of the priority concepts I have identified for my ATI. This concept is in the category of the physiological identity on the NCLEX-RN test plan. This assignment will focus on basic comfort and care using the NCLEX-RN test plan in the nursing practice. The purpose of this paper is to highlight the effective application of basic comfort and care to impact the patient outcome, patient care, identify barriers to effective care, evidence-based practice, and interdisciplinary team.

Importance of Basic Comfort and Care to the Nursing Practice

Nurses always assess the needs of patients to aid in developing an effective plan of care and provide basic comfort and care that would reduce risks (Choi et al., 2020). This concept is essential in the nursing profession as it allows nurses to develop customized care and comfort depending on patients. Nurses must help the patients use assistive devices and help in the patient’s daily activities. This duty implies that nurses need to be close to their patients and design effective care that improves patient outcomes. Basic comfort and care include various activities such as managing pain, assessing the condition of the patient daily, assessing the skin integrity, and identifying the need for further medication with the help of other healthcare workers.

Increased care and comfort are required in caring for older adults. These adults are prone to various physiological changes such as poor eyesight, reduced hearing, poor judgment, increased risk of infection, and a special diet (Yamada et al., 2015). These activities require increased attention among the nurses to offer basic comfort and care to these adults. The physiological conditions need to be considered in planning for adult care. EBP shows that prioritizing patient care as it improves patient outcomes. Nurses need to adhere to the provided evidence-based practices in optimizing the quality of care. Limited comfort and care given to older adults would put them at increased risk of health complications (Groves et al., 2021). For example, they would have an increased risk of falls, malnutrition, and infection, among other health complexities. Therefore, nurses need to prioritize comfort and care to heighten patient outcomes.

Healthcare Disparities, Inequalities, and Interventions

Disparity and inequality are issues that the healthcare systems have fought since time immemorial. These issues are influenced by various factors such as gender, age, race, and economic status. Racial prejudice affects the care offered to patients as nurses would tend to offer quality care to patients of their race while offering poor care to those of different races (Yamada et al., 2015). The ANA nursing codes of ethics are in place to govern the conduct of nurses and eliminate inequality and disparity. These codes act as the principles of justice in healthcare, ensuring that each patient gets quality care irrespective of race, gender, or ethnicity. The language barrier is potential inequality and disparity related to basic comfort and care. The difference in language between the nurse and the patient might warrant a patient less care because of the compromised communication.

The solution to these barriers needs to start by training the nurses on the legal impact they would face by violating the ANA codes. Educating nurses on disparities in healthcare delivery has an impact on them learning effective measures of meeting the needs of all clients irrespective of their age or ethical background (Choi et al., 2020). Nurses should as well be trained to keep their own beliefs at the minimum and follow the standards of nursing practice. In cases of a language barrier, nurses need to request a translator to facilitate effective communication. Educating patients on their rights would also be important in designing the quality of care offered by the nurses.

Legal and Ethical Considerations and Intervention Challenges

Prejudice can cause a nurse to treat a patient differently, violating the ethical principle of justice in healthcare. Beneficence is a principle of ethics that prompts one to be doing good always. Beneficence is applicable in the nursing practice as it requires nurses to be doing to patients (Langhammer et al., 2018). If a nurse fails to meet the needs of the patient may result in various complications and harm, leading to the violation of the beneficence principle. Besides, if a nurse performs an activity that would improve the health condition of a patient and that leads to injury or even death of the patient, the nurse would face legal implications. They could face disciplinary action for negligence, lawsuit, and revoking of their operating licenses.

Nurses sometimes face an ethical dilemma where the family of the patient or the patient refuses to undertake standard care that would have a positive impact on their health. For instance, patients and their families might choose to withhold tube feeding, which is a basic comfort and care required to the patient (Choi et al., 2020). In such cases, the nurse needs to find a professional directive from the interdisciplinary team on the decision to be made in such a dilemma. Seeking expert opinion in such cases also limits nurses from participating in an activity that would affect their entire nursing profession.

Participants and Interdisciplinary Approach

Effective care and comfort across healthcare organizations require the input of the healthcare team, such as nurses, doctors, dieticians, therapists, and other healthcare team members. The joint collaboration between these professionals leads to an improved decision-making process that further results in a positive patient outcome. Nurses always assess, prioritize patient care and offer assistive personnel (K, W. G. W, n.d.). On the other hand, doctors treat and offer effective treatment options that each patient needs. Therapists assess the musculoskeletal function of the clients by offering physical theory to the patient with compromised mobility.

Other members of the team outside nursing, such as dietitians, would assess, plan and offer effective education plans on the nutritional needs of these patients (Langhammer et al., 2018).These assessments are important to the older population who require a special diet to maintain their health and limit the risk of facing other risky infections.

Quality Improvement

The benefits of offering basic care and comfort are on the patient outcome. The improved care would heighten the patient-nurse relationship that allows the patients to share confidential information with the nurses to improve the health care services. It also affects cost-effective care because the patients can design better home-based care that would save on the amount of money used in visiting the hospital, medication, and other health care costs (Choi et al., 2020). Besides, nurses would as well feel motivated in their professional practice when there is improved safety, increased job satisfaction, and reduced cost of care.

Improved treatment and education of patients on the basic comfort and care principles heightens self-care and reduces hospital readmission rates. Nurses often serve as advocates of patient care and can sometimes challenge the doctors on an action they feel can harm the patient (Groves et al., 2021). They can always offer direct feedback, which is important in improving the quality of treatment and care. The promotion of professional nursing practice should align with continuous education to help nurses keep up with the trend and evidence-based practices in the healthcare system.

Conclusion

Basic comfort and care provide patient support with daily activities and promote health and wellness. It improves patient outcomes, further decreasing the stay of patients and the hospital, and heightening the recovery process. However, nurses need to eliminate barriers to care, such as bias and injustice. The value of the interdisciplinary team should be utilized to allow nurses to solve ethical dilemmas.
