Select a special population (e.g., pediatrics, geriatrics, pregnancy, lactation). Select a route of administration (e.g., oral, IV, subcutaneous, IM, transdermal, rectal, inhalation, SL) and discuss the advantages and disadvantages of this administration route. Give an example of a medication administered by your chosen route and a good patient candidate or rationale for choosing your given route of administration. Take into consideration guidelines that apply to medication administration in your special population (e.g., Beer’s criteria, pregnancy and lactation categories) and discuss why you selected these guidelines. Discuss the issue of polypharmacy in the population you select. Include the name of the medication in the subject line so that the medications can be followed. Make sure you choose a different medication from your peers. Include references in APA format.

NURS 304 -Pharmacology

The transdermal route of drug administration is a form of delivering a drug into the patient’s body system through the skin. Transdermal drug delivery system (TDDS) has recently emerged to be the most preferred route of drug administration especially among the elderly, as a result of its reduced rejection rate, ease of administration, great convenience, and reduced side effects such as pain on the injection site among IV administered drugs (Jeong et al., 2021). Compared to other routes of drug administration, TDDS displays very minimal risk of infection. Other advantages of TDDS include no GI side effects and minimizing adverse events from peak plasma concentration. However, this route of drug administration also displays some limitations such as prolonged period of administration, increased risks of burn, changes in the skin structure, and use of large concentrations of the drug.

Among the elderly population, TDDS is considered a good alternative route of drug administration as a result of several comorbidities and polypharmacy. Conditions such as cardiovascular and respiratory disorders are based managed by drugs administered through the oral and IV route as a result of the short onset of action and prolonged duration of action. However, most elderly patients are also faced with neurological complications, such as Alzheimer’s disease and depression (Jeong et al., 2021). To avoid an increased risk of adverse drug reactions as a result of polypharmacy, several transdermal formulations such as lidocaine patch 5% and capsaicin 8% patch have been developed to help in the management of these neurological conditions among the elderly. For instance, the United Healthcare community plan has provided a ‘clinical pharmacy program guideline for Lidocaine patch’ (Khan, & Roberts, 2018) This guideline provides comprehensive information regarding the use of lidocaine patches among the elderly, supported by the American Academy of Neurology. This makes the information valid and relevant to use in clinical practice.
