As innovators, nurse practitioners serve as change agents responsible for driving processes and policies and leveraging technology to provide better, safer, and more affordable care to community members. The Agency for Healthcare Research and Quality Guide to Adopting Innovations recommends the use of scholarly sources to make an evidence-based decision regarding the potential efficacy and appropriateness of a particular product to fill a certain gap in practice(Ross-Hellauer et al., 2020). Interdisciplinary collaboration is also a crucial aspect in broadening the perspective of the nurse essential in the process of introducing the new practice. The purpose of this project is to focus on the use of the chronic care model in the development of an evidence-based self-management support program for a dialysis patient to improve patient compliance and decrease morbidity and mortality rates among CKD patients.

 

Format/ Forum Selected for the Scholarly Product for Dissemination

Dissemination can be referred to as the planned process involving the consolidation of the target audience and the setting where the research findings are meant to be received, in addition to the communication and interaction among a broader service and policy audiences in the manner that facilitates research to promote the decision-making process. However, the adoption of an appropriate product planning guide is necessary for promoting the synthesis of the information to be disseminated (Mannay, 2019). For this project, the scholarly product development process will follow ten steps, such as writing a statement of purpose to affirm the project rationale; identification and prioritization of the target audience; identification of the primary product in addition to the desired sub-products; identification of the distribution of medium for the product; development of a tentative strategy for dissemination; anticipating the training needs of the user; anticipating testing of the product; scheduling revision of the product; identifying resource needs; establishing the timelines of the project, and lastly determining evaluation method.

However, at this point, our main focus is on the forum product selected for dissemination, which is the use of abstract. Abstracts of Forum products are usually converted into ‘web page product announcements’ by the National Center for Education Statistics (NCES) (Battaglia & Glasgow, 2018). One main advantage of the abstract is that it allows the developer to get straight to the point without any fluff. It’s convenient to the readers as it is short and precise. Consequently, abstracts can be converted easily to product announcements and Forum bronchus.  On the contrary, the use of abstracts bears some disadvantages like the availability of little room to share the information which provides the reader a full understanding of the product. Constructing an abstract also requires high scholarly writing skills for one to be able to distill a large quantity of information like 100 pages, into a 150-word abstract.

Additional Ways to Disseminate the Findings

Other than abstracts,  the use of brochurescan be considered as an alternative. A brochure is a traditional way in which information is shared through paper, but contains less text as compared to other formats such as books. The main advantage of brochures is that it is easily manageable especially when the volume of information being sheared allows it, for both the reader and the developer (Conde-Caballero et al., 2019). Brochures are also easy to distribute whether by hand or by mail. Generally, the use of brochures is quite cheaper despite the demand for production expertise and professional development. On the contrary, the use of this tool bears some advantages like the ability to convey only limited information given the restriction of the number of pages. They are also less official as compared to books hence not long-lasting.

 Dissemination and Personal Development

As a scholar-practitioner and a nurse leader, I shouldidentify gaps within my practice, and come up with an evidence-based solution through research. Once the necessary information has been acquired, it is crucial to identify the most appropriate manner in which this information can be shared with colleagues among other professional personnel (Barnes‐Daly et al., 2018). This is where dissemination comes in. It helps in the planning and consideration of the target audience. Generally, the dissemination process helps nurses promote their scholarly skills by making it easier to determine, who, where, and how the research finding and information synthesized will reach the target audience.

Conclusion

The most crucial aspect of research is how to communicate the findings. For instance, for the development of an evidence-based self-management support program for dialysis patients with CKD, it is crucial to select the most appropriate way of sharing the information with colleagues to promote its incorporation into practice. Dissemination thus helps in identifying the target population and finding the most appropriate forum through which the information will be conveyed. For this project, the selected forum is abstract, which is quite brief and simple to use and can be posted on the organization’s website for easy accessibility. Alternative approaches discussed include the use of brochures.

My Abstract project:
Chronic kidney disease (CKD) prevalence rates have been on the rise in recent years, and currently about thirty-seven million Americans are living with the disease. Most of the patients with CKD progress to end-stage renal disease (ESRD), which results in additional treatments such as renal replacement therapies, including kidney transplants and hemodialysis. Hemodialysis requires that the patients adhere to the treatment protocol and hospital visits as in the treatment plan. The gap in practice is that the dialysis center where this project will be implemented does not currently have a patient self-management support program. This project will bring together an interprofessional team to design an evidence-based self-management support program for dialysis patients receiving care in an outpatient dialysis center answering the practice-focused question by demonstrating that an interprofessional team can develop consensus an evidence-based self-management support program for dialysis patients that the dialysis center will approve for use. The theoretical model that will guide the development of the project is the chronic care model. A meaningful connection between patients and the health care team may increase patient compliance, decrease morbidity and mortality, and increase longevity.
