One of the differential diagnoses that I would consider for this patient is Addison’s disease. Addison’s disease is a metabolic disorder arising from adrenal insufficiency. The insufficiency results in the reduction in mineralocorticoids and glucocorticoids. Hormones that include androgens and aldosterone decline significantly. The consequences include sodium depletion and an increase in serum levels of potassium. Patients suffering from Addison’s disease develop symptoms that include fatigue, weakness, anorexia, and changes in ECG such as prolonged QT and PR intervals (Barthel et al., 2019).

The other differential that I would consider is hyperthyroidism. Hyperthyroidism is the elevation of thyroid hormones. Conditions that include Grave’s disease, thyroid cancer, and elevated secretion of thyroid-stimulating hormone cause this disorder. Patients present with complaints such as palpitations, tachycardia, anorexia, and fatigue (Léger & Carel, 2018). The last differential diagnosis is hyperkalemia attributed to secondary causes such as chronic kidney disease, excessive dietary consumption of potassium, and medications. Hyperkalemia causes changes in the ECG such as peaked T waves. Patients also present with symptoms that include muscle weakness, chest pain, and palpitations (Di Lullo et al., 2019).

The most important information to ask the patient includes her medication use history, dietary habits, and any history of diabetes. Information such as a history of thyroid gland disorders, menstrual cycle changes, kidney disease, and infections such as tuberculosis and fungal infections should be obtained as they increase the risk of metabolic disorders. Diagnostic testing is important to determine the cause of the client’s problem. The recommended diagnostics for the patient include thyroid function tests, complete blood count, liver and renal function tests, insulin-induced hypoglycemia tests, and serum potassium tests (Barthel et al., 2019; Di Lullo et al., 2019; Léger & Carel, 2018). The test will guide the development of accurate treatments for the patient in this case study.
