My client, a seventy-year-old female walks to the clinic because of concerns about drinking too much. My response to the client is that the National Institute on Alcohol Abuse and Alcoholism (NIAAA) provides the guideline for determining whether a person’s drinking pattern is within the acceptable range of problematic. Drinking patterns are categorized as moderate, binge drinking, or heavy drinking. Thus, I would explain to the client that a drinking pattern of more than three drinks per day or seven in a week for women amounts to heavy drinking (NIAAA, 2021). AJ divulges that she takes 2-3 glasses of wine a day and a martini before dinner. Wine has a higher content of alcohol (12%) than alcohol (5%); hence 2-3 glasses every day is heavy consumption (Zupan et al., 2018). Thus, the client is a heavy drinker.
To determine whether the client has an alcohol use disorder/dependency, I would use the Alcohol Use Disorders Identification Test (AUDIT) for screening. I will first explain the screening test, its purpose, and possible outcomes to the client. The evidence from the screening test is that the client has an alcohol dependency problem.
The client requests if they can take medication to help with the problem. My response is that alcohol use disorder is a combination of biological and psychosocial issues, requiring a combination of medication treatment and psychotherapy. The function of medication is simply for detoxification.Naltrexone and acamprosate are FDA-approved medications for treating alcohol use disorders (Kumar et al., 2020). However, for long-term outcomes, alcohol dependence treatment requires behavioral change, achieved through behavior therapies such as CBT, which helps a person develop coping skills and healthy thinking patterns that promote positive behavioral outcomes. Additionally, the 12 step Alcoholic Anonymous program will also help overcome the addiction.
