Patient safety is the selected clinical problem of focus. The Institute of Medicine (IOM) describes patient safety as avoiding harm on patients. Patient safety is identical to providing quality health care (Pelzang & Hutchinson, 2018). Greater emphasis is placed on healthcare delivery systems that prevent medical errors, learn from past errors, and build on a culture of safety involving health care providers, health organizations, and patients. Common patient safety issues include healthcare-associated infections (HAIs), medication, diagnostic, lab, and surgical errors, surgery associated complications, falls, and errors in identifying patients and communication (Pelzang & Hutchinson, 2018). Human and healthcare system factors are major contributors to these issues. Westat et al. (2014) note that as health organizations constantly seek to improve clinical practices, there is an increasing need to create a safety culture. Patient safety in delivering quality health services is a requirement to promote better patient outcomes and make progress towards achieving SDG 3 of promoting healthy lives and fostering health and well-being for all individuals across the lifespan.

Significance of the Problem In Terms Of Patient Outcomes

The World Health Organization (WHO) identifies unsafe patient care as a worldwide problem. Unsafe medical care is attributed to millions of injuries, morbidities, and mortalities globally, yet 50% of these detrimental outcomes are preventable through patient safety practices. The incidence of health complications secondary to unsafe medical care is a common cause of disability and mortality globally. According to the WHO (2019), medication errors are a common cause of harm and preventable injury in health care settings. Healthcare costs related to drug errors globally is approximately $ 42 billion annually. Besides, HAIs occur in 7 and 10 out inpatients. Risky transfusion procedures put patients at risk of transfusion reactions and contracting blood-borne infections. In addition, diagnostic errors are present in approximately 5% of patients in outpatient facilities putting patients at risk of severe harm and poor health outcomes (WHO, 2019). Most individuals will encounter a diagnostic error in their lifetime. Prevention of harm is a major element of patient safety and refers to freedom from accidental or preventable injuries resulting from medical care (Westat et al., 2014). Patient safety practices include interventions that seek to minimize the risk of adverse events attributed to exposure to medical care.

The PICOT question with regards to the clinical problem on patient safety is: “In patients living in nursing homes, how does involving a favorable patient safety culture improve patient’s safety as compared to the normal strategies in two months?”

The purpose of this paper is to describe the levels of evidence with regards to the PICOT question and search strategy used to retrieve literature sources.

Levels of Evidence

The PICOT is a therapy question since it probes the effectiveness of an intervention (involving a favorable patient safety culture) in enhancing patient outcomes (improving patient safety). Therapy questions are used to establish which interventions lead to the best outcome. The ideal evidence to answer the PICOT is a Randomized Controlled Trial (RCT). RCT is an experimental study whereby two groups are formed by a random process, and a blinded intervention is used. Therefore, an RCT will help answer the PICOT by randomly assigning two patient groups, an experimental and a control group. The experimental group will be cared for unfavorable patient safety culture, while normal strategies will be applied in the control group. The two groups will be evaluated for the one with better patient safety outcomes.

Search Strategy

Search terms used include “nursing home,” “patient safety culture,” “improved patient safety.” The first search retrieved over 60 articles; most were irrelevant to the PICOT question. The year of publication ranged from the 1990s to 2021. Besides, the articles were written in various languages. Databases used in retrieving the articles include the Chamberlain University library, PubMed, CINAHL, and OVID Medline. The search was refined by limiting the publishing year to articles published from 2014 and written in English. In addition, I refined the search to articles that explored patient safety culture to make them more specific to the PICOT question. Five articles were produced after refining the search.

Relevant Articles

Reis et al. (2018) aimed at identifying studies that have utilized the Hospital Survey on Patient Safety Culture (HSOPSC) to obtain data on hospitals’ safety culture. The study also surveyed the findings from the studies’ in the dimensions of culture of safety and feasible interventions to improve overall healthcare. It established that creating and supporting a safety culture is an essential approach to reducing and managing healthcare risks in organizations. Lee et al. (2016) analyzed how various concepts of safety culture are connected with clinical handoffs and approaches to healthcare safety. The study found that constructive handoff of information, accountability, and responsibility were essential to positive approaches in patient safety.

Conclusion

Patient safety is a vital element of health care quality. There is a need to create a culture of safety in patient care setting to reduce the injuries, morbidity, and mortality rates associated with patient safety issues. The adverse effects of patient safety issues are widespread, affecting high, middle, and low-income countries. The PICOT question compares the impact of a favorable patient safety culture and normal strategies in improving patient safety. The selected articles establish that creating a safety culture can minimize patient risks in health organizations.
