External Ventricular Drain (EVD) is an example of a recently introduced healthcare technology for the care of adult patients with complex conditions. EVD temporarily drains CSF from the brain’s lateral ventricles or the spine’s lumbar space into an external collection bag (Chau et al., 2019). A patient is admitted to the ED with a severe head injury following a road traffic accident and has a reduced level of consciousness. He reports having a severe headache, blurred vision, and has difficulties in mobility and speech. In this case, an EVD will be used to measure intracranial pressure (ICP). If the patient has an elevated ICP, the EVP will be used to drain CSF and monitor the ICP status during hospitalization. The purpose of this paper is to explore EVD, including its therapeutic modality, risks and benefits, roles and responsibilities of the interdisciplinary team in caring for a patient with EVD, and patient education about EVD.

Explanation and Background

An EVD is a system used to drain cerebrospinal fluid (CSF) through gravity and intercerebral pressure. EVDs are used in neuro-critical care setting to measure and reduce elevated intracranial pressure (ICP). ICP is monitored through a fiber optic monitor placed on the brain’s surface, in the brain, or an EVD system (Chau et al., 2019). The CSF flows through the brain and spinal cord and is reabsorbed via the arachnoid villi. The EVD is also used to drain infected CSF in cases of meningitis and to drain bloody CSF or blood after hemorrhage or surgery. It drains CSF in patients undergoing open Thoraco-abdominal Aortic Aneurysm (TAAA) repair surgery or those who become symptomatic post-surgery. In addition, it is used to monitor the CSF flow rate.

The EVD works through a closed sterile system that allows the drainage of CSF through a silastic catheter tip that lies in the ventricle. A physician connects the drainage system and notes its level, and then a critical care nurse maintains the system. The drainage rate depends on the height at which the EVD system is positioned in relation to the patient’s anatomy (Chau et al., 2019). The management of a patient with an EVD necessitates constant assessment and intervention. When an ICP monitor is inserted in the OR, a patient must be closely monitored with regular post-anesthetic observations when admitted to the recovery room. However, monitoring ICP via the EVD is contraindicated in patients on anticoagulation therapy or with coagulation disorders and scalp infections.

Risks and Benefits

EVD has various benefits for patients, especially neurosurgical and critically ill patients. These patients often require monitoring of ICP, and the EVD has helped implement the intervention. Besides monitoring the ICP, an EVD has the additional benefit of treating hydrocephalus and lowering an increased ICP through cerebrospinal fluid drainage. Besides, regular radiographic imaging is usually an essential element of multimodal monitoring of a patient’s neurological status. Positive outcomes when using the EVD can be achieved by proper leveling of the EVD. To facilitate an accurate interpretation of its value, the physician/nurse should ensure that the EVD’s pressure transducer is maintained at the same horizontal position as the ventricles. Besides, the EVD should be re-leveled whenever the patient moves.

Despite the potential benefits, EVD is associated with various risks, which may result in complications related to placement and transportation. EVD insertion poses the risk of CSF leak and systemic infection. Intracranial infections such as meningitis and ventriculitis can occur due to non-sterile techniques, multiple insertions, failure to maintain a closed ventricular system, delayed replacement of a compromised EVD, and delayed EVD removal despite clinical indications (Lu et al., 2019). Furthermore, since patients usually regularly return to radiology for follow-up imaging and procedures, they are at high risk for secondary brain injury during transport (Stout et al., 2019). Complications can be prevented by applying aseptic techniques during insertion, prompt replacement of defective EVDs, and regularly assessing patients on EVD to determine if they should be removed. Patients should also be administered IV antibiotics to cover the skin flora when the EVD is in-situ (Lu et al., 2019). In addition, patients with an EVD must always be accompanied by a proficient nurse from the OR to the ward.

Interdisciplinary team’s Roles and Responsibilities

The interdisciplinary team members caring for the patient with an EVD include a physician, critical care nurse, radiologist, nutritionist, and respiratory therapist. The physician is tasked with inserting the EVD aseptically and monitoring the patient. The critical care nurse’s role is to manage the patient’s care by modifying ventilator settings, fluid, medications, and therapeutic devices to stabilize the patient during recovery (Reiter, 2021). The radiologist is tasked with conducting and interpreting follow-up imaging procedures. The ICU nurse and radiologist should plan and communicate before the patient is transported to radiology to reduce intra-hospital transport complications and promote vigilance in monitoring during transport (Stout et al., 2019). The nutritionist assesses the patient’s dietary needs and plans the diet plan to provide nutritional support. Besides, the respiratory therapist is responsible for respiratory physiotherapy, administering oxygen, and managing the ventilator.

As a member of the interdisciplinary team, the nurse acts as a care coordinator and patient safety advocate. The nurse should communicate with the team about the patient’s progress and any deterioration in health. Besides, the nurse coordinates care among the team members and ensures that each member is aware of their roles and implements them (Reiter, 2021). Lastly, the nurse has a role of advocating for the patient by ensuring that the interventions implemented do not pose the risk of harm or go against their cultural and personal beliefs.

Nursing Scope of Practice

The registered nurse (RN) caring for a patient with EVD must possess vast knowledge of brain physiology and how it changes as patients recover or deteriorate from high ICP. Besides, the RN should be well-versed with patient populations at-risk of increased ICP and the clinical features of elevated ICP (Sakamoto et al., 2021). This allows prompt identification and management of patients with an increased ICP. The RN should have EVD and CVAD competencies to allow them to sample CSF using standard aseptic techniques every 24 hours. In addition, the RN should possess skills such as the ABCs of managing increased ICP. The RN should be able to assess patients for changes in neurologic status, preserve cerebral blood flow by optimizing cerebral perfusion pressure, and protect the brain from secondary injury (Sakamoto et al., 2021). Lastly, the nurse should have a caring attitude and be willing to provide emotional support to patients and their families in a high-technology environment.

Patient Education

The RN should educate the patient and the family about the effects of the external environment and external stimuli on the ICP. The nurse should engage the family in plans to manage the stimuli to reduce ICP elevation. Besides, the patient and family should be cautioned not to clamp the EVD. The information will be taught using a lecture method, and brainstorming will be used to encourage creative solutions to the problem. Cultural considerations to teaching will include using a language that the patient and family understand or getting an interpreter if there are language barriers (Shahmari & Hasanpour, 2021). The educator should also consider the cultural ways of communicating to avoid using gestures that may be culturally offensive. Furthermore, cultural beliefs and customs with respect to sick persons will be integrated into the teaching activity. Evaluation will be done using the Teach-Back method to ensure the patient and family adequately understands the information provided (Yen & Leasure, 2019). The Teach-Back method will be applied by instructing the patient and family to explain in their words what they have learned.

Conclusion

Increased ICP is common in neurosurgical patients and those with conditions that cause loss of cerebral autoregulation. The EVD is used to measure ICP, temporarily drain CSF, and monitor patients with an elevated ICP. It helps relieve elevated ICP, drain infected CSF, and monitor CSF flow rate. However, EVD is associated with the risk of infections, CSF leak, and complications during the transport of patients. Thus, the nurse should be well-versed in brain physiology and have the knowledge and skills to identify and manage patients with increased ICP.
