A data model is a visual portrayal of the elements of a database. The most prevalent data model is the ERD (Entity Relationship Diagram). To appropriately build an ERD, it is critical first to determine what would form the attributes and entities. The cardinalities indicate the relationship between such entities (Yarlagadda & Syed, 2016). The data sets are classified (into Tables). In the provided case, the resultant ERD is the following: patients, labs, ordered tests, medical insurance, and doctors. The relevant cardinalities will demonstrate the link between the identified entities. There exist six cardinalities in a basic ERD framework (Gravelle, 2010). This will characterize the relations between the entities as many-to-many, many-to-one, one-to-many, one-to-one, zero-to-many, and one-to-zero.

In the given scenario, one doctor can serve many patients, but one doctor can help one patient at a time representing a one-to-one relationship. The doctor may order one or more tests to be done by the radiologist for every patient, but one test that may be requested is linked to one patient. This implies that the doctor and the test requested table will have a one-to-many relation. Lab test and ordered test have a one-to-many relation. In this situation, the lab test will also have a one-to-one relationship with the requested test. Because a doctor can only ask for a specific test, there may be a one-to-one association between lab test results and doctors. Each appointment has both a check-in and a check-out time. Because a particular insurance policy is linked with a single patient, patient and medical insurance entities will have a one-to-one relationship.
