Meet with your preceptor or SME to review the requirements for your CPE. Strategically plan the tasks and timeline needed to complete your CPE. You should perform a force field analysis of the forces that will facilitate or impede the proposed implementation of your HIP.

 

Create a force field analysis by completing the “Force Field Analysis Template.” Collaborate with interprofessional members of your project team, preceptor or SME, and faculty to complete the force field analysis. The force field analysis should be an investigation of the forces that will facilitate or impede the proposed implementation. The force field analysis should include the following:

 

·         three positive forces that may drive the proposed implementation of the HIP

·         three negative forces that may restrain the proposed implementation of the HIP

 

Discuss the results of the force field analysis with your project team, preceptor or SME, and faculty. Describe how knowing about each of the positive and negative forces will help you manage your project. Establish a plan to overcome the forces that are against the proposed implementation. Identify facilitators that can be used to support your HIP.  Discuss the force field analysis with your preceptor or SME.

 

Create a 5- to 10-minute GoReact video that discusses the results of your force field analysis. After recording your video and posting it for review, watch two of your peers’ videos and provide them encouraging and constructive feedback. Finally, capture a screenshot of your video and screenshots showing you responded to two of your peers’ videos.

Consult with your project team, preceptor or SME, and faculty to create a Gantt chart for your HIP. You may use a program of your choice to create the Gantt chart or this Gantt Template that works in MS Excel. The Gantt chart should include the following:

 

·         Tasks – this field lists your HIP tasks. Please list five high-level tasks here.

·         Assigned to – this field includes the initials of the person to whom each task is assigned. Please do not include actual names of people, only initials.

·         Progress – this field tracks the percent complete for each task (for example, 60% complete, 90% complete)

·         Start dates – this field tracks the start date of each task

·         End dates – this field tracks the end date of each task

·         Bar chart – the bar chart is a visual representation of the duration of each task

The graduate examines the structure, processes, and outcomes of an organizational plan that may facilitate or impede the implementation of a healthcare improvement project.

7073.04.4 : Implementation Planning

The graduate leads the development of a collaborative implementation plan that includes oversight of the project and the scope of work with specific tasks, timelines, and deliverables.

INTRODUCTION
To fulfill the requirements for this course, you must complete the attached “CPE Record.” You will need to electronically sign and date the record. Your supervising preceptor or SME must date, initial, and sign this form before you submit it for evaluation. You and your preceptor or SME should use DocuSign to sign the form (see web link below). Please see the “DocuSign Instructions” in the Supporting Documents section for more information.

Within the “CPE Record,” you will enter your name, as well as the name of your preceptor or SME. You will not sign the form at this time. Once you click “Finish,” DocuSign will send the form to your specified preceptor or SME to date, initial, and sign to verify you have met all of the requirements. After your preceptor or SME completes this portion, the signed document will be emailed to you to sign as well.

Finally, you must upload the “CPE Record” and the required deliverables from that record to your e-portfolio. You should complete this task before submitting JAM1 Task 1.

REQUIREMENTS
Your submission must be your original work. No more than a combined total of 30% of the submission and no more than a 10% match to any one individual source can be directly quoted or closely paraphrased from sources, even if cited correctly. The originality report that is provided when you submit your task can be used as a guide.

 

You must use the rubric to direct the creation of your submission because it provides detailed criteria that will be used to evaluate your work. Each requirement below may be evaluated by more than one rubric aspect. The rubric aspect titles may contain hyperlinks to relevant portions of the course.
