The identified topic for the capstone change proposal is Type 2 diabetes. Type 2 diabetes refers to a condition that is characterized by insufficient regulation of blood sugar. It is a complex and highly prevalent health condition that poses serious public health concerns because it is linked to other chronic diseases, morbidity, and premature mortality. According to the CDC (2022), diabetes affects nearly 37 million Americans. Type 2 diabetes can be categorized under community health concern because its effects do not only affect the patient and their families but also the entire community. The community characteristics that contribute to the risk of type 2 diabetes are access limitations and environmental characteristics, which make it more difficult to engage in lifestyle changes that prevent diabetes such as consumption of a healthy diet and engaging in physical activity. Moreover, the community has populations that are at high risk of developing this condition including the elderly and certain ethnic and racial groups including African Americans, Hispanics, Alaska Natives, and people of Asian or Pacific Islander descents.
Diabetes is a major community health concern that is associated with serious health outcomes such as hypertension, obesity, kidney diseases, and leg, nerves, and feet problems that pose a risk of amputation. The condition is also associated with adverse economic effects on the patients, their families, the health care system, and the government including loss of income, high cost of care, lowered productivity, high death rates, and substantial national budget allocation to address the condition (Cunningham et al., 2018). Therefore, it is imperative to address this community health concern in nursing practice to improve health outcomes among African Americans.

This capstone change proposal recommends the adoption of nurse-led diabetes self-management education (DSME) to address the condition (Lee, 2018). The proposed nurse-led DSME can be categorized under community intervention since it consists of educational sessions delivered to a group of people drawn within a given community. The nurse-led DSME intervention is also expected to address specific needs diabetic patients from various cultural and ethnical groups. The adoption of DSME will foster optimization of evidence-based practice to attain the needs of diabetic patients.
