Background

Childhood obesity is a serious health concern in the United States and globally. Obesity refers to the condition which is associated with excess accumulation of body fats to an extent of causing negative health effects. It is established by the body mass index (BMI). The rates of childhood obesity are remarkably increasing in the current society. In the United States, nearly 16.2% of the younger population aged between 10 and 17 years are suffering from obesity (State of Childhood Obesity, 2022). Globally, millions of children are obese or overweight and at risk of obesity. Childhood obesity is attributed to physical, social, and psychological problems, which can affect the child throughout their entire lives. Childhood obesity is among the major reasons for obesity during adulthood and the earlier risk of conditions associated with obesity in adulthood. Many children and adolescents are increasingly becoming overweight and obese due to sedentary lifestyles and engaging in more westernized dietary habits, which entail the consumption of foods with higher fat and sugar contents and low fiber content.

Problem Statement

Childhood obesity is a serious epidemic and among the leading public health and social issue in the United States and globally. Childhood obesity is associated with adverse and far-reaching effects such as ill-health and a decline in life expectancy rates among this population. Therefore, it needs to be addressed to prevent the escalation of its effects. According to Sanyaolu et al., (2019) children and adolescents with obesity are likely to suffer from various conditions including breathing issues such as asthma and sleep apnea and high cholesterol and high blood pressure, which are risk factors for heart diseases. Obesity is also attributed to a high risk of insulin resistance, diabetes, and risk of psychological and social problems such as poor self-esteem and discrimination. Childhood obesity is also associated with long-term effects such as high chances of becoming obese in adulthood and the associated chronic diseases such as cancer and hypertension (Sanyaolu et al., 2019). Other than the health issues, childhood obesity is also linked to serious economic impacts involving heavy expenditure on the condition.

Based on the increasing prevalence of childhood obesity and its short-term and long-term effects on health such as predisposition to cancers, hypertension, cardiovascular diseases, diabetes, and stroke, childhood obesity should be regarded as a serious public health concern. There are various strategies to address the issue of childhood obesity. The purpose of this capstone project is to address childhood obesity through the education program. This project advocates for the adoption of health education on lifestyle changes to reduce obesity rates among obese adolescents. This nursing intervention is expected to create awareness among children, adolescents, parents, schools, and the community to ensure a healthy lifestyle among obese adolescents and reduce obesity rates.

PICOT Question

This capstone project advocates for the adoption of educational initiatives by nurses to create awareness and inspire healthy lifestyles for adolescents at school, at home, and in community settings.  The PICOT question is, in obese adolescents (P) does health education on lifestyle changes (I) Compared to no intervention (C) reduce adolescent obesity rates (O) by 10% in 12 months (T)?

Literature Search Strategy Employed

A literature search for this capstone project was conducted using major databases such as PubMed, EBSCO, MEDLINE, CINAHL, Science Direct, Cochrane Library, and Proquest Central. These databases were selected based on their strength in providing high-quality journals in nursing and healthcare. They are also highly flexible and can accommodate sophisticated searches and filtering. Database search involved the use of key terms such as obesity, childhood obesity, adolescent obesity, lifestyle changes, health education, obese adolescents, nutrition education, dietary education, physical activity, and healthy lifestyle. These keywords were essential in generating articles that specifically focused on the nursing intervention. Initially, the literature search only focused on nursing journals. Later, medical journals and health care journals were employed in the search to provide background information. Consideration was put to the articles that focused on health education on lifestyle changes in childhood/adolescent obesity. The literature search used articles published between the years 2018 and 2022 to provide current statistics and information.

Evaluation of the Literature

Evaluation of literature in this project was done using the literature evaluation table (LET) provided by the GCU. The LET is comprised of definite predetermined areas to be evaluated. The areas entailed the authors’ name, name of the peer-reviewed journal, permalink or working link to access the article, article’s title, year of article publication, research question or hypothesis, study purpose, study design, setting or sample, methodology, analysis, key findings, study recommendations, and how the article supports capstone project. All the studies used in the LET were evaluated based on the criteria provided by the LET.

Applicable Change or Nursing Theory Utilized

The applicable nursing theory in this capstone project is Orem’s Self-Care Deficit Theory. According to Gligor & Domnariu (2020), this theory aims to enhance nursing quality and is premised on the principle that an individual should spearhead his or her self-care through engaging in active roles in performing activities aimed at maintaining good health. The theory also acknowledges that self-care is impacted by primary conditioning factors such as family, age, and gender. As such, nurses should not hold patients entirely responsible for their health but strive to ensure that they improve their care and health. In the project at hand, the health education program on lifestyle changes will empower obese adolescents to develop new perspectives on marinating good body weight and health making them take charge of their personal care and ultimately improving their health.

Proposed Implementation Plan with Outcome Measures

The implementation plan of this project involves various steps. The first step is the identification of project participants and asking for their informed consent. The subsequent step is to conduct needs assessments to identify project facilitators and issues that participants may face. After the needs assessment, the project objectives are developed and the context of the education program, which is the proposed intervention, is selected. The context of intervention comprises vital factors such as needs evaluation, hypothetical background, seeking professional advice, and conducting systematic reviews to develop a sophisticated change solution. The final step is the classification of the context of change and evaluation of the intervention. This evaluation will determine whether or not the project will be implemented. If the evaluation demonstrates that the nursing intervention is feasible and appropriate, the project will be implemented.

How Evidence-Based Practice was used in Creating the Intervention Plan

EBP is critical to the success of this project by ensuring that project decisions are based on the best existing evidence obtained from advanced research studies. Nurses are expected to rely on evidence-based approaches when developing and implementing the education program in this project. Nurses can also use EBP to establish the level of learning in patients, identify proper teaching techniques, evaluate the progress of learning, and establish the appropriateness of the education program (Jolley, 2020). Eventually, the success of the education program relies on the use of reliable EBP.

Plan for Evaluating the Proposed Nursing Intervention

The proposed nursing intervention will be evaluated based on the outcomes of the project. An evaluation will be conducted on the data collected and the baseline information following the implementation of the project. If the evaluation demonstrates improvements in measures such as reduction in obesity rates, the intervention will be regarded as successful. Moreover, different tools such as follow-ups, interviews, and questionnaires can be utilized to evaluate the proposed nursing intervention.

Identification of Potential Barriers to Plan Implementation and how to overcome them

This project is likely to encounter various probable barriers. The first challenge is the possibility of limited knowledge among nurse educators to offer effective health education to obese adolescents. This barrier can be addressed through the provision of proper training for nurse educators. Another potential barrier is organizational, social, cultural, and structural obstacles. These barriers can be addressed through the formulation of proper policies and the evaluation of the existing structures. Finally, the project can potentially be limited by ineffective stakeholder support. This barrier can be addressed by effective, open, honest communication among stakeholders.

Conclusion

The purpose of this capstone project is to address childhood obesity through the education program. Childhood obesity is a serious epidemic and among the leading public health and social issue in the United States and globally. Childhood obesity is associated with adverse and far-reaching effects such as ill-health and a decline in life expectancy rates among this population. A literature search for this capstone project was conducted using major databases such as PubMed, EBSCO, MEDLINE, CINAHL, Science Direct, Cochrane Library, and Proquest Central. Evaluation of literature in this project was done using the literature evaluation table (LET) provided by the GCU. Orem’s Self-Care Deficit Theory was used to guide the project. The project also used an elaborate plan for the implementation and evaluation of the intervention. EBP will highly be used to guide the implementation of the project.
