Medication errors is a prevalent patient safety issue that impacts healthcare outcomes and patient care experience with increased cost burden. The Food and Drug Administration (FDA) defines a medication error as any preventable event that leads to inappropriate use of medications by healthcare professionals which in some instances leads to patient harm. In their article, MacDowell et al. (2021) assert that medication errors entail a failure in one of the five “rights” of the medication administration process. These include right patient, right medication, right time, right dose, and right route. The purpose of this paper is to offer an annotated bibliography and summary based on research associated with best practices in addressing medication errors faced by health care organizations.

Medication Errors

Healthcare providers need to address the issue of medication errors because of the negative effects that these events have on patient care safety, patient care experience and overall outcomes from health care interventions. Despite efforts to reduce medication errors, many facilities, departments, and units in hospitals continue to face increased prevalence of these adverse events. Close to 25% of medication administration situations lead to medication errors. Close to seven million patients get affected by medication errors each year which leads to increased cost of care due to longer hospital stay and poor patient care experience> Medication errors cause more deaths than traffic accidents, auto-immunodeficiency syndrome and cancer of the breast.  I have witnessed medication errors in my organization when nurses give the wrong medications due to limited use of barcode spanning to ensure that the right patient gets the right amount of medication using the right route and the right time with requisite dosage. As a nurse involved directly in patient care, I have a responsibility to ensure that I go through the medication administration process to mitigate medication errors.

The most appropriate way in confirming relevance and the credibility of the sources is to ensure that the date of publication is recent and captures the issue under study. Imperatively, the selected articles for this annotated bibliography were those within the last four years. I also reviewed each article for possible intervention approaches to decrease medication errors and promote safe administration of medicines by healthcare professionals.

In this article, the authors conduct a systematic review to identify and examine barriers that hinder nurses from reporting medication administration errors in hospital situations. The article bases its objectives on the need by the World Health Organization (WHO) to reduce medication-associated harm by 50% over a five-year period beginning in 2017. Using an integrative review of articles from different databases that include Cumulative Index to Nursing and Allied Health Literature (CINAHL), PubMed and Web of Science among others, the authors identify barriers that hamper efforts to report medication errors by nurses in their facilities. These include organizational barriers like insufficient reporting systems, management behavior and individual and professional barriers like personal reasons, lack of knowledge about errors and fear of colleagues’ reprisal. The authors recommend the need for an enabling environment to prevent punitive measures and a culture of blame that are imperious for nurses to report these events. The article is relevant to medication errors since it evaluates issues that hinder effective reporting of these adverse events to have corrective measures to prevent their occurrence.

The article introduces medication errors as one of leading global healthcare issue. The authors assert that medication errors occur in all phases of treatment that include copying, prescribing, preparation, checking the medication, administration and observation. The authors advance that close to 38% of the errors come from administration of medications. The hypothesis is that medication administration consist of the highest percentage of medication errors. Therefore, registered nurses (RNs) who are the last people involved should have an active role in addressing this global healthcare challenge in different care settings. The authors attribute medication errors to systematic and professional failure by both providers and healthcare systems. The article emphasizes the need for better communication, staff education, modification of policies and protocols, and use of inter-professional and multidisciplinary collaborations to reduce medication errors and promote patient safety. The article is essential to my topic of interest since it provides an account on the existence of medication errors and offers approaches to help maintain quality of care and safety.

In this article, the author provides examples of medication errors and the possible solutions to reduce them in healthcare settings. The article discusses near-miss errors and events that involve use of antibiotics, vancomycin, and vecuronium as neuromuscular blocking agent. The article provides a case study of patient with medication errors emanating from the use of vecuronium instead of the prescribed vancomycin.  Due to effective processes, the providers identified the error before administration. An inquiry into the event showed that the storage of these two medications was the problem. The article suggests that effective storage in appropriate and well labeled shelf or area can mitigate this error in the future. This article is relevant to the topic of interest because it offers a real life situation of a medication error happening. However, through effective quality and process controls and protocols, the organization was able to detect the error. It also took the necessary steps to reduce and prevent the possibility of another event like this one.

At the core of this article is the correlation between barcode medication administration and reduction of medication errors. The researchers assert that barcode scanning of medications prevent wrong patient, medication, dose, time and route errors through checking an administered medication to align with the medication administration records. The authors emphasize the need for monitoring nurse scale rates to ascertain if they are actually scanning all medications as required. The authors also assert that the use of on wheels’ workstation, medication dispensing system and barcode scanning assists in reduction of medication errors. The article is appropriate for the topic of medication errors because it show that leveraging technology can reduce these events and ensure that patients get the best care. barcode scanning is a protocol that nurses should use to enhance patient safety by reducing occurrence of adverse events. It is imperative that nurses use set systems correctly to mitigate and prevent medication errors while promoting safety and efficiency.

Learning from the Research

Researching articles on medication errors provides a lot of insights and learning for me as a nurse practitioner focused on developing effective interventions for better care delivery. One critical lesson is that peer-reviewed journal articles are a fundamental aspect of effective research evidence as offer credibility and relevance to the topic under study. I have learned that while medication errors persist in healthcare provision, healthcare professionals can take a host of interventions to reduce them, prevent their occurrence and severity to patient outcomes, safety, and care experience. The research demonstrates the critical role nurses play in quality care delivery and patient safety. The annotated bibliography has shaped my knowledge perspective about research and gives a clear understanding of the need to avoid medication errors as the best way to guarantee patient care quality.

Conclusion

Medication errors are a leading cause of poor health outcomes for patients in different situations. As illustrated by the articles in this paper, nurses should take an active role to attain quality and safety standards for patients through effective mitigation of medication errors. The implication is that healthcare organizations should work and leverage technology to improve medication administration errors.

In 2010, the Federal Department of Human and Health Service (DHHS) launched the Healthy People 2020 goals to include a goal to eliminate health inequality/disparity. Healthy People 2020 defines a health disparity as “a particular type of health difference that is closely linked with social, economic, and/or environmental disadvantage.
